What are your office hours?

Our clinics are open Monday through Friday, from 8:00 AM to 5:00 PM. If you need assistance outside these hours, please contact us at 888-927-3227.

Do you accept walk-in patients?

We do not accept walk-in patients. However, we offer same/next-day in-person or telehealth appointments with our medical experts.

How do I schedule an appointment?

What insurance plans are accepted?
We are participating providers in a growing number of health insurance plans. To find the complete list of health insurance plans we are in-network with, please visit www.BariatricInstitute.com/insurance.
 
If you have questions about your specific coverage, please call our Patient Financial Services Department, which will help you with insurance eligibility and verification.
What should I bring to my appointment?
To ensure your appointment goes smoothly, please bring the following items with you:

  • A valid government-issued photo ID for identification purposes

  • Your insurance card, if you have health insurance

  • Any relevant medical records or test results that may help us understand your health history

  • A list of all current medications, including dosages and supplements

  • A list of any questions or concerns you’d like to discuss during your visit

If this is your first visit, arriving about 10-15 minutes early to complete any necessary paperwork is highly recommended. This will help us start your appointment on time and provide you with the best possible care.
 
If you have any special needs or require accommodations, please let us know ahead of time so we can prepare accordingly.

Is parking available?
Many of our locations offer free, convenient on-site and street parking; however, parking availability is not guaranteed and may be subject to a fee.

What should I do if I need urgent care outside of business hours?

In the event that you require urgent care outside our business hours, we advise you to immediately seek care at the nearest emergency department.

For any life-threatening emergencies, immediately call 911.

How do I request copies of my medical record?
To obtain a copy of your medical records, you must submit a formal written request to our Medical Records Department via mail, fax, or in person.
 
Your request should include your full name, date of birth, current contact information, and a valid form of identification. You must also sign the medical record release form.
